MEDICARE GLOSSARY

What does Step Therapy mean?

Step therapy is a treatment plan for people who frequently take prescription medications to manage a medical problem like high blood pressure, asthma, or arthritis. It enables your employer to keep providing prescription drug coverage while also enabling you and your family to get the necessary therapy at an affordable price. Step therapy, also known as "fail-first" therapy or sequential therapy, is a cost-control strategy used by health insurance companies to manage medication utilization. It involves a process where patients are required to try less expensive or preferred medications before they can access more costly or non-preferred alternatives.

In step therapy, the treatment journey typically begins with the prescription of lower-cost or generic drugs, which are considered the first step in the therapy sequence. These medications are often widely accepted as effective and have a lower price compared to other options. The patient must demonstrate that they have tried and failed to achieve the desired results with the initial medication before they can move on to the next step.

If the initial medication fails to produce the desired outcome or causes adverse side effects, the patient can then seek approval to "step up" to the next medication in the sequence. This process continues until the patient finds a medication that proves effective for their specific condition.

The goal of step therapy is to promote the use of cost-effective treatments while minimizing unnecessary expenses for both patients and insurers. By requiring patients to try and fail with lower-cost alternatives first, insurers aim to ensure that more expensive medications are only prescribed when they are truly necessary. It is important to note that step therapy can sometimes pose challenges for patients who may have already tried and failed on certain medications or who require a specific treatment due to their medical condition. However, in some cases, exceptions can be made through prior authorization processes or appeals, allowing patients to access the appropriate medication without having to go through the entire therapy sequence.
