We respect your privacy and want you to know how we collect and share information about you.

When you call us, you will be calling a licensed AHIP-certified (Medicare) state-specific licensed health insurance agent. When you speak with our agent, they will need the state and zip code you will be residing in to determine which plans are available in your area. Per Medicare-mandated compliance, the licensed health agent must begin the conversation by requesting you sign a federally mandated form by the Centers for Medicare and Medicaid Services (CMS) (and thus federally required) named the "Permission to Contact" and "Scope of Sales Appointment" forms. If this form is not signed by both the beneficiary and agent, the agent would be required to terminate the conversation.

Per Federal Law and Medicare Regulations, these singular or combined forms must be signed by both the consumer and insurance agent prior to discussing any of the potential client's health needs. They are only used for the sole purpose of discussing health insurance topics that are on that particular form(s).

If the beneficiary refuses to sign the form, that's your choice, but the conversation regarding health insurance or Medicare cannot legally be started or responded to by the insurance agent until the form(s) are signed by both client and agent.

This form only allows the agent to discuss the different health care plans, and that is your only commitment. You're only agreeing to allow the agent to discuss Medicare or related and approved CMS health plans. You're not required to use their services. The "Scope of Sales Appointment" and "Permission to Contact" forms are federal regulations, and we are required to follow a certain order when speaking with you, the potential beneficiary, either in person or over the phone or computer.

These forms are HIPPA-compliant, and we do not share your personal health information without your consent.

By signing the Scope of Sales Appointment and Permission to Contact form, you, the beneficiary, are not contractually tied to the agent in any way, shape, or form. This form only allows the agent to discuss the different health care plans that are available to the consumer.
