Medicare prescription drug tiers

Medicare and Medicaid have prescription drug tiers that cover various drug price ranges for various drug groups. You will typically pay less for a medicine in a lower tier than one in a higher tier. As of April 2023, depending on what your lifetime Medicare out-of-pocket expense has been for Medicare Part D, there may or may not be an out-of-pocket deductible cost for Tiers 1-2 prescription drugs using a Medicare Advantage plan.

Medicare prescription drug plans (Part D) generally divide covered medications into different tiers based on their cost and coverage level. There is no standardization for how different carrier plans categorize drug formularies into tiers, but most Part D plans use a system with four or five tiers.

The most common tier structure for Medicare Part D plans is:

    Tier 1: Preferred Generic Drugs: These are the least expensive drugs on the plan's formulary (a list of covered drugs), and they typically have the lowest cost-sharing amounts for the beneficiary.

    Tier 2: Generic drugs: These are non-preferred generic drugs that are still less expensive than brand-name drugs but may have slightly higher cost-sharing amounts than Tier 1 drugs.

    Tier 3: Preferred Brand-Name Drugs: These are brand-name drugs that are included on the plan's formulary, and they typically have higher cost-sharing amounts than generic drugs but are still less expensive than non-preferred brand-name drugs.

    Tier 4: Non-Preferred Brand-Name Drugs: These are brand-name drugs that are not included on the plan's formulary or are in a higher cost-sharing tier. They typically have higher cost-sharing amounts than Tier 3 drugs.

    Tier 5: Specialty Drugs: These are high-cost drugs used to treat complex, chronic, or rare conditions. They typically have the highest cost-sharing amounts of all the tiers.

It's important for beneficiaries to review their plan's formulary and tier structure carefully to understand the specifics of their coverage and cost-sharing amounts for their prescription drugs.
