Medicare health Insurance Secondary payer

Under certain circumstances, Medicare may serve as a secondary payer for healthcare services when another insurance plan is the primary payer. This is known as the Medicare Secondary Payer (MSP) program. The Medicare Secondary Payer (MSP) program applies when another insurance plan, such as an employer-sponsored health plan, is responsible for paying for healthcare services before Medicare does. In these cases, Medicare may pay the remaining costs after the primary insurance company has paid. The MSP program may apply in the following situations:

  • The insurance coverage, strategy, or program that reimburses a claim for medical care comes second. Depending on the circumstances, this can be Medicare, Medicaid, or another type of insurance.
  • Group health insurance: Medicare may be a secondary payer if the individual is covered by an employer-sponsored group health plan that has 20 or more employees.
  • Workers' compensation: Medicare may be a secondary payer for medical expenses related to a work-related injury or illness.
  • Automobile or liability insurance: Medicare may be a secondary payer for medical expenses related to a car accident or other injury caused by a third party.
  • No-fault insurance: Medicare may be a secondary payer for medical expenses related to a personal injury that is covered by no-fault insurance.
